Acetonaemia (Ketosis) in buffaloes is a metabolic disorder seen during early lactation due to negative energy balance, when energy demand for milk exceeds intake. Mobilization of body fat leads to excess ketone production. Risk factors include poor appetite, low-energy diets, and concurrent diseases. Clinical signs include dullness, lethargy, sharp milk yield drop, anorexia (especially for concentrates), weight loss, firm dry feces, and a characteristic sweet acetone odor in breath, milk, or urine. Severe cases may show nervous signs like circling and aggression. Diagnosis is based on history, clinical signs, and tests: urine/milk ketone detection (dipsticks, Rothera’s test), blood hypoglycemia, and elevated BHB. Differential diagnoses include displaced abomasum, mastitis, metritis, or indigestion, but ketones confirm ketosis. Treatment includes IV 50% dextrose, oral glucogenic precursors (propylene glycol, sodium propionate), corticosteroids (dexamethasone), vitamin B complex, and dietary adjustments to improve energy intake and manage underlying causes.
