An ideal antimicrobial agent possesses several key properties, ensuring effective and safe treatment. Foremost is selective toxicity, meaning it should harm the pathogen without significantly harming the host. It should have a broad spectrum of activity, being effective against a wide range of common pathogens, or a narrow spectrum for targeted treatment to minimize resistance. The agent should be bactericidal (killing bacteria) rather than merely bacteriostatic (inhibiting growth), especially in immunocompromised patients. It must be able to reach the site of infection in therapeutic concentrations and remain stable in the body. Ideally, it should have a low incidence of resistance development, be non-allergeniceasy to administer, and cost-effective. It should also not interfere with the body's natural defense mechanisms.

Antimicrobial agents are broadly classified based on several criteria:

  1. Spectrum of Activity:
    • Broad-spectrum: Effective against a wide range of gram-positive and gram-negative bacteria (e.g., tetracyclines).
    • Narrow-spectrum: Effective against a limited range of pathogens (e.g., penicillin G primarily against gram-positives).
  2. Mechanism of Action:
    • Inhibition of cell wall synthesis: Targets bacterial peptidoglycan synthesis, leading to lysis (e.g., penicillins, cephalosporins, vancomycin).
    • Inhibition of protein synthesis: Binds to bacterial ribosomes, disrupting protein production (e.g., tetracyclines, aminoglycosides, macrolides, chloramphenicol).
    • Disruption of cell membrane function: Damages the bacterial cell membrane, altering permeability (e.g., polymyxins).
    • Inhibition of nucleic acid synthesis: Interferes with DNA or RNA replication/transcription (e.g., fluoroquinolones, rifampin).
    • Inhibition of essential metabolic pathways: Blocks crucial enzymatic pathways (e.g., sulfonamides, trimethoprim).
  3. Effect on Bacteria:
    • Bactericidal: Kills bacteria directly (e.g., penicillins, aminoglycosides).
    • Bacteriostatic: Inhibits bacterial growth, allowing the host immune system to clear the infection (e.g., tetracyclines, sulfonamides).
  4. Chemical Structure:
    • Beta-lactams: Penicillins, cephalosporins.
    • Aminoglycosides: Gentamicin, streptomycin.
    • Tetracyclines: Doxycycline, oxytetracycline.
    • Macrolides: Erythromycin, azithromycin.
    • Fluoroquinolones: Enrofloxacin, ciprofloxacin.
