The correct answer is

Someone has changed the batteries in the smoke detector.

Identify the tense. ‘Have been changed’ is the present perfect passive. Its active counterpart is the present perfect active: has/have + past participle.

Supplying the agent. The passive sentence names no doer, so the active version needs an indefinite subject — someone. Being singular, it takes has, giving ‘Someone has changed the batteries in the smoke detector.’

Why the others fail — all three keep the right voice but the wrong tense:

‘Someone had changed…’ — past perfect, which places the action before some other past event and so alters the time frame.

‘Someone has been changing…’ — present perfect continuous, which suggests an action still in progress rather than one completed.

‘Someone changed…’ — simple past, which reports a finished event without the present relevance the perfect carries. This is the closest wrong answer: the batteries were changed in both readings, but only the perfect says that they are now changed.

Takeaway: voice change must preserve tense exactly. Name the tense of the given sentence before looking at the options.
