Per the Keith-Wagener-Barker classification, Grade 1 shows mild arteriolar narrowing and Grade 2 shows more marked narrowing with AV nicking, without haemorrhages or exudates. Flame-shaped haemorrhages and cotton-wool spots appear from Grade 3 onward, and Grade 4 additionally has papilledema. Hence only grades 3 and 4 show these findings.
