Describe differences between adult and fetal hemoglobin and comment on their physiological significance.

Adult hemoglobin (HbA) and fetal hemoglobin (HbF) are specialized to meet the oxygen transport needs of their respective stages of life. HbA, present in adults, consists of two alpha and two beta chains (α2β2) and is adapted to efficiently pick up oxygen in the lungs, where oxygen pressure is high, and release it in tissues, where oxygen pressure is lower. In contrast, HbF, the predominant hemoglobin in fetuses, contains two alpha and two gamma chains (α2γ2). The gamma chains interact less strongly with 2,3-bisphosphoglycerate (2,3-BPG), a molecule that normally decreases oxygen affinity. This reduced interaction allows HbF to bind oxygen more tightly, shifting its oxygen dissociation curve to the left, and enabling the fetus to extract oxygen effectively from maternal blood across the placenta, even under lower oxygen conditions. After birth, HbF gradually declines as HbA production ramps up, ensuring oxygen delivery is optimized for extrauterine life. This difference is vital for fetal development and smooth transition to independent respiration postnatally.
