Small intestine

Bile is delivered into the small intestine — option 3, specifically into its first part, the duodenum.

The route. Bile is produced continuously by the liver, stored and concentrated in the gall bladder, and released when food arrives. It passes down the common bile duct, which joins the pancreatic duct, and the two open together into the duodenum at the ampulla of Vater, guarded by the sphincter of Oddi. The hormone cholecystokinin, released by the duodenal wall when fatty food arrives, is what makes the gall bladder contract.

What bile does. It contains no digestive enzymes at all. Its work is twofold. The bile salts emulsify fat, breaking large globules into a fine suspension of tiny droplets and so multiplying the surface area on which the pancreatic enzyme lipase can act. And bile is alkaline, so it neutralises the acid chyme arriving from the stomach and brings the contents to the slightly alkaline pH at which the intestinal and pancreatic enzymes work. Bile also carries away bilirubin, the pigment from broken-down haemoglobin, which gives faeces their colour.

A clinical note. If the duct is blocked — commonly by a gallstone — bilirubin accumulates in the blood and produces jaundice, and fat digestion is impaired.
