Which one of the following statements is true regarding neurovascular anatomy of the uterus?

UPSC CMS 2026 Surgery, Gynaecology & Obstetrics, and PSM Question Paper (02-Aug-2026)
The correct answer is

The venous channels of the uterus drain into the internal iliac veins.

The uterine venous channels drain into the internal iliac veins. Thin-walled, valveless uterine venous plexuses lie alongside the uterus in the broad ligament, communicate freely with vaginal, vesical and rectal plexuses, and drain via the uterine veins into the internal iliac veins. The absence of valves explains the ready spread of pelvic sepsis and thrombophlebitis. (The ovarian plexus drains separately — right ovarian vein to the IVC, left to the left renal vein.)

Why the others are wrong: The uterine artery arises from the anterior division of the internal iliac, not the external iliac (which gives only inferior epigastric and deep circumflex iliac branches). It crosses the ureter about 1.5 to 2 cm lateral to the internal os, not 3.5 cm — the classic "water under the bridge" relation and the commonest site of ureteric injury at hysterectomy. Uterine pain travels with sympathetics through the inferior hypogastric plexus to T11 and T12 (body) and S2 to S4 (cervix), so it is referred to the hypogastrium and back, never to T5 to T6 (nipple level).

Key point: Fix three facts: uterine artery from the anterior division of the internal iliac, crossing the ureter about 2 cm lateral to the internal os, uterine pain referred to T11 to T12.
