Microscopic Agglutination Test (MAT)
The Microscopic Agglutination Test (MAT) is considered the gold standard for diagnosing Leptospirosis. This test detects antibodies against specific Leptospira serovars.
- Sensitivity & Specificity: MAT is highly sensitive and specific, especially when performed after the first week of illness.
- Definitive Diagnosis: It is often used retrospectively to confirm infection by observing a significant rise in antibody titres between acute and convalescent serum samples.
Comparison with Other Tests
While other tests can aid in diagnosing Leptospirosis, they are not typically considered the gold standard:
- ELISA (Enzyme-Linked Immunosorbent Assay): Detects antibodies (IgM and IgG). It's useful for early detection but can sometimes lack serovar specificity compared to MAT.
- PCR (Polymerase Chain Reaction): Detects Leptospira DNA. It's valuable for early diagnosis (first week) directly from blood or urine but is technically demanding and less common than serological tests.
- Blood Culture: Directly cultures the bacteria. It's highly specific but has low sensitivity, especially after the first week of illness, and requires specialized laboratory conditions.
Therefore, due to its specificity and ability to identify serovar-specific antibodies, MAT remains the gold standard diagnostic test for Leptospirosis.
