The correct answer is

Madhya Pradesh

Madhya Pradesh — option 3, as recorded in the Commission’s key.

Why the question needs care. Chitrakoot straddles a state boundary. The pilgrimage region lies partly in the Satna district of Madhya Pradesh and partly in the Chitrakoot district of Uttar Pradesh, with the Mandakini river running between. Most of the principal shrines — and the town of Chitrakoot Dham itself — are on the Madhya Pradesh side, which is the answer the key follows.

SiteSignificance
RamghatThe bathing ghats on the Mandakini, the centre of the pilgrimage
KamadgiriThe sacred hill circumambulated by pilgrims, held to be the abode of Rama during exile
Hanuman DharaA spring on the hillside with a Hanuman shrine
Sphatik Shila, Janki Kund, Gupt GodavariSites associated with the exile
Bharat Milap templeWhere Bharata is said to have met Rama to ask him to return

The tradition. Chitrakoot is where Rama, Sita and Lakshmana are said to have spent eleven and a half of their fourteen years of exile. It figures in the Ramayana, in Kalidasa’s Raghuvamsha, and in Tulsidas’s Ramcharitmanas — Tulsidas himself is associated with the place. The name means “the hill of many wonders”.

Do not confuse it with two similar names. The Chitrakote Falls on the Indravati in Bastar, often called the Niagara of India, are in Chhattisgarh — which is why option 1 is offered. And Chitrakoot should not be confused with Chitrakut in other contexts; the Odisha option has no such site at all.

A modern association : the place is also the site of the Deendayal Research Institute’s rural development work and of Chitrakoot Gramodaya Vishwavidyalaya, India’s first rural university.

Hence, the answer is Madhya Pradesh.
