RRB NTPC 2015 CBT 1 Question Paper PDF (2-Apr-2016) (Shift 3)
The correct answer is

Both Assertion and Reason are true, and Reason is the correct explanation of Assertion.

The assertion is true: medicines should be taken only on a doctor's prescription.

The reason is also true: a doctor knows what a medicine actually contains, which dose is safe, and what side effects or reactions it can cause, while a person taking it on their own usually does not.

This danger of not knowing a medicine's ingredients and side effects is exactly why medicines should not be taken without a prescription, so the reason correctly explains the assertion.

Hence, Both Assertion and Reason are true, and Reason is the correct explanation of Assertion.
