The manager, as well as his assistants, was late.

Option 2 is wrong because 'prevent' is followed by 'from + ing', so it should read 'He was prevented from going out by the guard'.

Option 3 is wrong because 'look forward to' is a phrasal verb where 'to' is a preposition and must be followed by a gerund: 'I look forward to meeting you soon'.

Option 4 uses the structure 'X, as well as Y'; the verb agrees only with the main subject 'The manager', which is singular.

Therefore the singular verb 'was' is correct, and the parenthetical phrase 'as well as his assistants' does not change the number of the verb.

With correct subject-verb agreement and no other faults, option 4 is fully correct.
