Working backwards. Turning an indirect question into direct speech reverses every change made when it was reported.

1. Reporting verb. Inquired is past, so in direct speech it becomes said — also past. The tense of the reporting verb never changes.

2. Conjunction and punctuation. If is dropped, the words go inside quotation marks, and a question mark is restored.

3. Word order. The reported clause used statement order (‘his name was’). A direct question restores question order: ‘Is your name…’

4. Tense and pronoun. The backshifted was reverts to is, and his becomes your, since the question is addressed to the person concerned.
