Rh Incompatibility and Hemolytic Disease of Newborn
Haemolytic disease of the newborn (HDN), specifically Erythroblastosis Fetalis in the context of the Rh system, occurs under specific circumstances related to blood group incompatibility.
Condition for HDN Development
HDN arises when an Rh- negative mother carries an Rh+ positive fetus. This situation typically develops when:
- The mother (wife) is Rh-negative.
- The father (husband) is Rh-positive.
If the father is Rh+, there is a significant chance the fetus will inherit the Rh+ factor, leading to an Rh+ fetus developing inside an Rh- mother.
Mechanism of Incompatibility
During pregnancy or delivery, fetal Rh+ red blood cells can enter the mother's Rh- bloodstream. The mother's immune system recognizes the Rh+ factor as foreign and produces antibodies against it. These antibodies can cross the placenta in subsequent pregnancies and attack the fetus's red blood cells, causing haemolysis (destruction of red blood cells), leading to HDN.
Therefore, the core requirement for this condition is the incompatibility between the Rh blood group systems of the husband (father) and the wife (mother), specifically when the mother is Rh- and the father is Rh+.
