During active labor, a woman experiences sudden respiratory distress, hypotension, and cyanosis. What is the most likely diagnosis?
Suspect amniotic fluid embolism
Amniotic fluid embolism occurs when amniotic fluid and fetal cells enter the maternal circulation during labour or delivery, triggering an anaphylactoid reaction.
Its classic picture is sudden respiratory distress, cyanosis, hypotension and cardiovascular collapse, often followed by DIC.
Cord prolapse causes fetal distress, placental abruption causes painful bleeding and a tense uterus, and uterine inversion occurs in the third stage with shock and haemorrhage — none fits this sudden respiratory picture.
Hence, Suspect amniotic fluid embolism.
