The International Federation of Gynecology and Obstetrics (FIGO) staging system for cervical cancer classifies the disease based on clinical assessment and diagnostic procedures. Understanding which procedures are mandatory is key for accurate staging.

Mandatory Staging Procedures

Certain procedures are considered essential components for the clinical staging of carcinoma of the cervix according to FIGO guidelines:

• Pelvic Examination: This is crucial for assessing the size of the primary tumor, identifying any involvement of the vaginal walls, parametria, pelvic sidewalls, and checking for enlarged pelvic lymph nodes. It forms the basis of clinical assessment.
• Biopsy: Histological confirmation of squamous cell carcinoma or adenocarcinoma is required. This is typically obtained via punch biopsy or colposcopically directed biopsy.
• Endocervical Curettage (ECC): Recommended when the primary tumor is not visible on the ectocervix or if there is suspicion of endocervical involvement. It helps determine the extent of tumor invasion within the cervical canal.

Non-Mandatory Procedure for FIGO Staging

While valuable for patient management, certain investigations are not considered direct requirements for the initial clinical FIGO staging itself.

• Ultrasound abdomen: Imaging techniques like ultrasound are often used to evaluate the extent of disease, assess for lymph node involvement, or check for distant metastases. However, ultrasound findings are generally supplementary to the clinical staging criteria defined by FIGO and are not inherently mandatory for assigning the FIGO stage. Staging relies primarily on direct clinical examination and biopsy results.

Therefore, Ultrasound abdomen is the procedure that is NOT a mandatory part of the FIGO staging criteria for carcinoma of the cervix.
