• Brawny swelling of the sub-mandibular region: This indicates a firm, woody swelling, characteristic of diffuse cellulitis and tissue hardening seen in Ludwig's angina.
• Inflammatory oedema of the mouth: Swelling within the mouth, particularly the tongue and floor of the mouth, is a hallmark sign, potentially leading to airway compromise.
• Putrid halitosis: Foul breath often accompanies these infections, stemming from necrotic tissue and bacterial activity, frequently linked to an underlying dental source.

Differential Diagnosis Consideration

While other conditions involve neck or facial swelling, Ludwig's angina is specifically characterized by the rapid, bilateral progression into the submandibular and sublingual spaces, accompanied by significant oral oedema and foul odour.

• Tubercular adenitis typically presents as chronic, often less tender lymph node enlargement.
• Chronic lymphadenitis involves persistent but usually less acute swelling.
• Acute lymphadenitis is painful node swelling but less commonly presents with the diffuse, brawny floor-of-mouth involvement and severe halitosis described.

The combination of severe oral oedema, submandibular swelling, and putrid halitosis points strongly towards Ludwig's angina as the most likely diagnosis.
