Most Common Site for Pressure Sore Development

Pressure sores (decubitus ulcers) occur when unrelieved pressure over a bony prominence compresses the skin and underlying soft tissue against bone, compromising local capillary blood flow and causing ischemic tissue damage. In immobile or debilitated patients who spend most of their time lying supine or semi-recumbent, the sites bearing the greatest and most prolonged pressure are affected first and most frequently.

Why the Ischium Is the Most Common Site

• Occiput: This is a recognized pressure point in patients lying supine for prolonged periods (e.g., unconscious or ventilated patients), but it is a far less common overall site than the ischium because most patients are not kept flat on their backs continuously.
• Heel: The heel is an important and common pressure point, especially in supine, immobile patients with poor peripheral circulation, but epidemiologically it ranks below the ischium and sacrum in overall frequency of pressure ulcer development.
• Greater trochanter: This site is prone to pressure sores mainly in patients positioned in prolonged lateral (side-lying) posture. It is a recognized site but is less commonly involved than the ischium, since most debilitated patients spend more time supine or seated than strictly side-lying.
