Assessing Intracranial Pressure Signs

The most common and sensitive early indicator of rising ICP is a change in the level of consciousness. This can manifest as:

• Drowsiness or lethargy
• Confusion
• Difficulty waking up
• In severe cases, progressing to stupor or coma

This change occurs because increased pressure impairs overall brain function and perfusion.

Evaluating Other Potential Signs

While other signs can occur with increasing ICP, they are often not the earliest or most common:

• Pupillary changes (like ipsilateral pupillary dilatation): This typically indicates brain herniation, specifically compression of the oculomotor nerve (CN III), which is a more advanced sign than a simple change in consciousness.
