Fasciotomy is a surgical procedure indicated to relieve pressure in compartment syndrome, a condition where swelling or bleeding increases pressure within a fascial compartment, impairing blood flow and nerve function. The decision for fasciotomy is based on clinical signs and objective measurements.

Indications Analysis

We examine each option to determine if it represents an indication for fasciotomy in compartment syndrome:

• Distal sensory disturbance: This indicates nerve compromise due to ischemia, a critical sign of compartment syndrome. It is a strong indication for fasciotomy.
• Compartment pressure > 30 mm Hg: This is an objective measurement indicating significant pressure within the compartment, suggesting impaired perfusion. It is a key indication for surgical decompression.
• Pain on passive movement of affected muscles: This is a classic clinical sign of muscle ischemia within the compartment. It suggests significant compartment syndrome and warrants fasciotomy.
• Palpable distal pulses: The presence of palpable distal pulses typically indicates that arterial blood flow is still adequate and has not been critically compromised by the elevated compartment pressure. While compartment syndrome can occur with palpable pulses, their presence alone is usually not considered a primary indication for emergency fasciotomy, especially when compared to the other signs listed.
