Uterine Fundus Position Post Third Stage Labour

Immediately following the delivery of the placenta (completion of the third stage of labor) in a full-term pregnancy, the uterus contracts firmly. This contraction helps control bleeding and marks the beginning of uterine involution (the process of returning to its pre-pregnancy size).

Assessing Fundal Height

The position of the uterine fundus is assessed by palpation. Right after the third stage of labor is completed:

• The uterus is typically well-contracted.
• The fundus is generally found at or very close to the level of the maternal umbilicus (navel).

This anatomical landmark indicates the expected state of the uterus immediately postpartum.

Analysis of Options

• At the level of xiphisternum: This position is too high and is typically seen late in pregnancy before labor begins.
• At the level of umbilicus: This is the correct and expected position of the uterine fundus immediately after the delivery of the placenta.
• Below the level of umbilicus: This position occurs later in the postpartum period as involution progresses over the subsequent days.
• Just above the symphysis pubis: This location corresponds to the size of the uterus around 12 weeks gestation or immediately after birth before significant contraction occurs.

Therefore, the uterine fundus is typically located at the level of the umbilicus immediately after the third stage of labor in a full-term delivery.
